Community Rehab: Student and Provider Information

VIEW PARTICIPANT INFORMATION

Community Rehab Alliance is an innovative service and workforce development strategy providing student-assisted community based rehab to people who have neurological or age-related progressive conditions in North West Queensland.

Community Rehab Alliance is based in Mount Isa at James Cook University’s Murtupuni Centre for Rural and Remote Health (MCRRH). Community Rehab Alliance is a collaboration between NWHHS, North West Remote Health (NWRH) and Gidgee Healing.

What we do:

  • Provide high quality and evidence-based rehab to the people of North West Queensland;
  • Provide an inter-professional service staffed by Clinical Lead Occupational Therapist, Indigenous Allied Health Officer and a range of allied health students. Additional clinical education support may include Exercise Physiology, Physiotherapy, Speech Pathology, Nutrition and Dietetics, Pharmacy, and Social Work from partnering services;
  • Develop the local workforce; and
  • Support allied health students through 5, 7 and 10 week clinical placements grounded in inter-professional practice, service learning, neurological rehab and remote practice

Community Rehab Alliance provides intensive student-assisted rehab blocks throughout the year including individual and group-based programs designed to meet local need.

 Intensive student-led block offers:

  • Individual programs (provided at home, centre or via telehealth)
  • Group programs that target the goals of people referred to Community Rehab Alliance. These will vary from block to block depending on the current rehab needs, but may include:
    • Mama’s and Papa’s health ageing group for Indigenous Elders
    • Ready and Steady - a fall prevention program (in partnership with NWRH)
    • Skills to Enable People and communitieS - a self-management group for people with stroke or brain injury (in partnership with ABIOS (Acquired Brain Injury Outreach Service)
    • Memory group
    • Hand & Arm group

Referrals

Participants can be referred by their GP, hospital, nurse, allied health professional or self-referral. Once received we will contact the referrer or participant within 10 working days.

If needs are identified with a participant that are best met by another local service provider Community Rehab Alliance will make contact regarding this and coordinate an onward referral. This may include: NDIS funded services, home modifications, assistive technology and specialist services i.e. vocational rehabilitation, vestibular assessment, driving assessment. 

Community Rehab Alliance provides rehabilitation to people who:

  • Who have a neurological condition e.g. stroke, spinal cord injury, head injury or Parkinson’s disease
  • Who are experiencing age-related changes e.g. falls, loss of balance, change in independence, or who need some help to keep active in their home or community
  • Who have had a change in their everyday abilities because of illness, disease or injury e.g. car accident, broken hip, long hospital stay
